This section provides more information on Insurance and Billing, Patient Forms and Frequently Asked Questions, which includes information on medical records requests, appointments, procedure expectations, lab results, Hepatitis C and Capsule Endoscopy.

Your initial appointment is to assess your current health status, review your medical history, perform a physical examination and formulate recommendations. Please allow approximately one hour for this appointment. Prior to your arrival to the appointment, please complete the new patient forms (which can be found on our Patient Forms page) and either fax them to (936) 321-0032 or bring them with you. In addition, please bring your driver’s license and insurance card(s). It is also important that you arrange for our office to receive, before your visit, any records of tests or procedures that may pertain to your visit. Make certain you have obtained any referrals that may be required by your insurance company or your appointment will be rescheduled.

Appointments are confirmed 48 hours in advance. Patients who do not keep their appointment and have not cancelled at least 48 hours prior to an appointment or procedure will be subject to a “no show” fee of $50 for office visits and $100 for procedures.
