Insurance and Billing
At the time of your visit, patients need to provide the front desk with a current insurance card and driver’s license to be copied. HMO and some other “managed care” plans require a referral from your primary care physician for specialty care. If your insurance company requires a referral and you do not bring a referral with you, we will reschedule your visit.
Patients will be responsible for full payment of any co-payments, co-insurance, and deductibles required by their insurance company. Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurance and non-covered services. We accept cash, check, Visa, MasterCard, American Express and Discover. We offer the option to pay bills online. Read instructions for paying online using our secure server here.
Appointments are confirmed 48 hours in advance. Patients who do not keep their appointment and have not cancelled at least 48 hours prior to an appointment or procedure will be subject to a “no show” fee of $50 for office visits and $100 for procedures.
